When possible, all patients will be seen
by appointment. Although we strive to stay
on schedule, as with any physician,
emergencies and patients with complex
problems may cause delays. When this occurs,
we request your patience and understanding,
but if you are unable to wait we will be
happy to reschedule your appointment at a
more convenient time.
Cancelling
Appointments
If you are unable to keep your appointment
or will be unavoidably delayed, please
notify the office as soon as possible. This
will allow us to better serve all of our
patients. Please cancel at least 24 hours in
advance.
Insurance
We will assist you in filing your
insurance by providing two copies of the
necessary medical information and charges
before leaving the office. Our form contains
sufficient information to submit for benefit
payments from all insurance companies.
Regardless of insurance benefits available,
the patient remains financially responsible.
Accounts not showing any payment in ninety
days will be considered delinquent. Please
feel free to discuss the fees with your
physician.
Managed Care
Plans
We participate in certain Managed Care Plans
and will file claims for you. However, it is
the patient's responsibility to see that we
have the most current insurance information
and referrals if required.
Regardless of insurance benefits
available, the patient remains financially
responsible. Accounts not showing any
payment in ninety days will be considered
delinquent.
Medicare
We do accept Medicare assignment and will be
happy to file any supplementary insurance as
long as the patient provides us with the
necessary information.
Fees
Payment is expected at time of service.
(This includes your co-pay or deductible).
If you require a copy of your charges we
will be happy to send that to you in the
mail. If you have a problem concerning your
account, please notify our business office.
Telephone Calls
We do not make diagnoses by telephone, but
questions regarding treatment or medication
may be called to the office staff during
regular office hours. If we have not seen
you for one year or more, we must reconfirm
the diagnosis before approving further
refills. No routine refills will be filled
on the weekend.
Emergencies
We will make every effort to see emergency
patients as soon as possible. Our answering
service can be reached after business hours
by calling the office phone number.
Laboratory Testing
Certain procedures require the services of a
pathologist. You will be billed separately
for these charges.
